Fresh muscle biopsy is snap frozen using liquid nitrogen and spectrum of tests including enzyme histochemistry is performed. This is required in the diagnosis of muscle diseases, especially mitochondrial disorders, congenital myopathies, muscular dystrophies, inflammatory and metabolic myopathies.

Immunohistochemistry on fresh frozen muscle is performed for the diagnosis of Duchenne’s muscular dystrophy, Becker’s muscular dystrophy, Sarcogylcanopathies, Dysferlinopathy, and Emery-Dreifuss muscular dystrophy

How should the sample be sent?

The biopsied muscle should be wrapped in saline moistened guaze (take care NOT to soak the guaze in saline) without any other fixative and immediately transported to the neuropathology lab. If the transport is time consuming, kindly place the muscle wrapped in saline moistened guaze in an airtight polythene pouch and place it in a thermosflask containing ice. Take care NOT to soak the biopsied muscle in ice water. The fresh muscle biopsy should reach the neuropathology lab in above mentioned condition within 12 hrs of biopsy for a meaningful interpretation. The biopsy should be accompanied with complete clinical details and any other relevant investigations.

Muscle biopsy fixed in either formalin or glutaraldehyde is analysed using light microscopy. However, enzyme histochemistry and immunohistochemistry for muscular dystrophies are NOT possible on the fixed muscle tissue.

How should the sample be sent?

The biopsied muscle should be placed in adequate amount of 10% buffered formalin/ 2.5% glutaraldehyde immediately after biopsy. DONOT place the biopsy in refrigerator. The biopsy should be accompanied with complete clinical details and any other relevant investigations.

Nerve biopsy fixed in either formalin or glutaraldehyde is analysed using light microscopy.

How should the sample be sent?

The biopsied nerve should measure atleast 2.5cm in length and should be placed in adequate amount of 2.5% glutaraldehyde / 10% buffered formalin immediately after biopsy. DONOT place the biopsy in refrigerator/ send it in saline. Glutaraldehyde/ cidex is preferred over formalin. The biopsy should be accompanied with complete clinical details and any other relevant investigations.

Tissue biopsy for immediate diagnostic services may be sent without any fixative/ saline immediately to the lab. The tissue will be used to make squash smears to help in immediate diagnosis of the lesion and to help guide the subsequent treatment. The report of the frozen/ squash smears will be provided within 30 min to 1 hour of receiving the sample

Kindly call atleast a day prior to the day of surgery to inform the need for frozen report

Tissue biopsy should be placed immediately after the biopsy in adequate amount of 10% buffered formalin. The tissue will be processed and stained with hematoxylin and eosin and examined under light microscopy. The additional special stains will be performed as per the need of the case. The reporting of the cases will be done in 5-7 working days as per the complexity of the case.

Immunohistochemistry required for diagnosis of tumors will be performed as per the need of the case. The immunohistochemistry panels that will be performed are: Glioma panel, metastasis panel, basic lymphoma panel, pituitary adenoma panel and others. Individual IHC markers may be performed as well. Immunohistochemistry will require additional 3-5 days for reporting of the cases.

Autoimmune antibody profiling is done using indirect immunofluorescence (IIF) and immunoblots (kits from Euroimmun, Germany). ANA, ANCA and autoimmune encephalitis profile is done via IIF. Immunoblots are used for ANA and inflammatory myositis profiling.

These tests will be performed once a week.
